Norwood's Outreach Support Program is a home-based rehabilitation program which aims to provide medium-term (12-24 months) planned intensive rehabilitation for people with a mental illness.

This program provides support to people who desire to live independently in the community with minimal support. An individual support plan, created for you, will identify your requirements.

The support provided by us may range from 1-2 hours per week or for more intensive support 4-6 hours per week.
