Norwood’s Pathways Program focuses on people who are experiencing, or are at risk of experiencing, homelessness. Working with the client, the main aim is to create a pathway into more stable and appropriate accommodation, as well as establishing support to sustain housing.

The program is designed to assist people who are at risk of homelessness at the point of discharge from an acute care facility within our catchment area.
